The Homewatch CustomCare process
Step 1 CustomCare™ Intake:
Following your inquiry call, the Homewatch CareGivers Intake Coordinator arranges an appointment at the potential client's residence. The assessment is a FREE service and is set at a time convenient for the client and/or family and other interested parties.
Step 2 CustomCare™ Consultation and Evaluation
During this initial meeting, the needs and requirements of the client, and the client's general health and condition are identified. In addition, a Home Safety Evaluation is made, identifying any significant concerns that should be remedied for the sake of the client and Homewatch CareGivers staff. A Fall Risk Assessment is also completed. You will also receive a FREE Evaluation and Assessment.
The Intake Coordinator then explains the in-home living assistance services, and suggests a Care Plan. Most often, the discussion results in a mutually acceptable Written Care Plan.
Step 3 CustomCare™ Written Care Plan
The Written Care Plan is a dynamic document that outlines the specific services, schedules, client preferences, and special instructions. It provides common communication to all parties involved in the care of the client, to maximize consistency and quality.
Step 4 CustomCare™ Care Service Agreement
The Homewatch CareGivers Agreement is signed only after the client and other involved parties have received and thoroughly understand the Client's Rights and Responsibilities including schedules, services and pricing.
Step 5 CustomCare™ CareGiver Selection
Once the Written Care Plan and the Homewatch CareGivers Care Service Agreement are signed, the company does its best to assign the optimum caregiver(s). The caregiver Manager matches the new client with the best caregiver, based on personalities, skills, expertise, availability, and interests. Our Perfect CareGiver Match™ guarantees we will do our best to find a suitable caregiver. The caregiver Manager reports on the status and well-being of the client to interested parties, as agreed upon by the client and Intake Coordinator.
Step 6 CustomCare™ CareGiver Orientation
Homewatch CareGivers supplies our caregivers with: a copy of the approved Written Care Plan, directions and other information concerning the client's care, and report forms that are regularly maintained. CareGivers are not sent to the client's home until they completely understand the client's needs, requirements and any special circumstances.
Step 7 CustomCare™ Quality Assurance
Homewatch CareGivers' Quality Control Team makes scheduled and unscheduled visits to the client's home while the CareGiver is present to evaluate: the assistance being provided, the changing needs of the client, and the client and caregiver relationship.
In addition, the caregiver Manager periodically reviews each client's Care Plan to make sure that the client's needs are best served. If changes or improvements are deemed appropriate, they are immediately addressed.
All personnel are willing and eager to meet with clients, family members, and other interested parties, whenever requested, regarding the provision and scope of services, schedule changes, and any other concerns.
